HEALTH COORDINATION POSITIONS

The Abbe Center for Community Mental Health has multiple positions open in a new Integrated Health Home:
• Nurse Care Manager – BSN or RN required.
• Care Coordinator – BA Degree in a human services related field.
• Peer Health Coach - Must have “lived experience” with a mental health diagnosis.
The Integrated Health Home provides coordination of all health related services needed by individuals with a serious mental illness.  Duties will include working as a part of a treatment team which integrates behavioral and physical health care.  Emphasis is on wellness and health promotion.

We are seeking individuals with excellent communication skills to work in a progressive community based program coordinating services for individuals with chronic and persistent mental illness.  These applicants must have a bachelor’s degree in social work, nursing, or human service field.  Experience working with Medicaid Waiver Programs required.
